SASSA Grant Affidavit Forms

SASSA Affidavit for Old Age Grant (Annexure I)

You have to confirm that you are the primary caregiver or biological parent of the child and that the child resides with you.
An affidavit also confirms that the child should not reside in any state institution and that the parents are not getting paid to take care of the child. SASSA Affidavit for Care Dependency Grant (Annexure I-5)

You, as an applicant, must confirm that you are the child’s parent, foster parent, or primary caregiver and that the child resides with you and not in any state-funded institution. SASSA Affidavit for Disability Grant (Annexure I-2)
A disability grant is offered to South African citizens who are between the ages of 18 and 60. To prove your eligibility for this grant, you have to submit an affidavit. The affidavit confirms your details, like name, ID, and place of residence. Your marital status and income sources also need to be shared.
If you and your spouse don’t have any income sources, then mention how you both are currently surviving. Also, declare the assets you and your spouse own. Finally, get this affidavit signed and stamped by the commissioner, and submit it after your thumb and signature.
SASSA Affidavit for Grant-In-Aid (Annexure I-7)
Grant-In-Aid is offered to the disabled person who needs someone to take their full-time care. With this affidavit, your eligibility is checked. Your details are confirmed. The details include your name, phone number, place of residence, and marital status.
You also have to confirm if you are receiving an old age grant, a war veterans grant, or a disability grant. If you live in any state-funded institution, then mention it. After completion of the form, get it signed and stamped by the commissioner, and submit it.

SASSA Consent form for bank payment
If you, as a SASSA grant beneficiary, want to receive your grant payment directly in your bank account instead of getting cash, then you have to submit the bank consent form. In this form, you have to provide your personal details and bank account details. With this form, you confirm that the information you provided is true and correct, and as the true account holder, you give consent to the given conditions without prejudice.
Along with this form, you also have to submit other documents, like a bank statement or proof of account. Sign the form and submit it.
SASSA Reapplication or reconsideration form
If your SASSA SRD grant application or any other grant application is rejected and you are unhappy with this decision, then you can request reconsideration. Using this reapplication form, you can mention why you disagree with the decision of SASSA. You also need to attach other documents in support of this application.
SASSA Proof of School Attendance form
A proof of school attendance form must be completed if the child is going to school. When you apply for the child support grant, you have to submit this form if the child is between the ages of 7 and 18. The child’s details are confirmed via this form. School authorities also have to share the child’s progress in school along with their attendance.
Get the school principal’s signature, school stamp, and SASSA stamp, and submit this form with your grant application.
SASSA Medical Assessment Referral Form
If you are applying for a disability grant, child support grant, grant-in-aid, or any other grant in which your medical condition is checked, you have to submit the medical assessment referral form. This form has 3 parts: A, B, and C. Part B of this form must be filled out by the treating clinician or institution.
SASSA Appeal Forms
If your grant application is rejected and your reconsideration request is also rejected, then you can submit the appeal to the Independent Tribunal for Social Assistance Appeals (ITSAA) by filling out this form.
Lodging of an Appeal Form: Form 1 (Version: June 2022 onwards)
Fill out this form if your application is rejected and you want to appeal. You have to mention the reasons why you disagree with the decision of SASSA. Submit this form along with the necessary documents to appeal.
SASSA Appeal Form 3 (Version 2017)
This is an old appeal form, and it was valid until the appeals were made before June 2022. If your appeals are after June 2022, then use the above form. Give the reasons why you disagree and submit the form along with the required documents to your nearest SASSA office.
Application for Condonation for Late Appeal – Form 4
You have to submit the appeal within 90 days from the date you received notification. If, for any reason, you missed the deadline and didn’t submit the appeal on time, then you have to fill out a condonation form for an extension to submit the appeal.
